VLER - Virtual Lifetime Electronic Record

Do You See a Non-VA Health Care Provider? 

"Connect Your Docs" through the Virtual Lifetime Electronic Record (VLER) Health Exchange program. This program gives VA and non-VA health care providers secure access to certain parts of your electronic health record. This access reduces the need for Veterans and their families to request and carry paper medical records from one health care provider to another. It also provides other potential benefits to Veterans and their providers.

This program is free and voluntary for Veterans.

If you have a completed Authorization form (VA Form 10-0485) to submit locally, please mail to:

Online: Visit https://www.ebenefits.va.gov and complete the following steps:

  1. Click on ACCESS MY DOCUMENTS at the top of the page. 
  2. Scroll down to the Health section and click MANAGE MY AUTHORIZATIONS AND PREFERENCES (VLER).
  3. At the bottom of the page click on SHARE YOUR ELECTRONIC HEALTH INFORMATION WITH NON-VA HEALTH CARE PROVIDER.
  4. Highlighted on this page you will find the Authorization form, the "Stop Sharing" form and a list of participating VA Medical Centers.

In Person: Visit the VLER Health Coordinator at VA Central Iowa in the Administrative Building (Bldg 3) in room 1964A, or the nearest CBOC to fill out the VA Authorization form (VA Form 10-0485)
