Group Advanced Care Planning Now Offered at VACIHCS
Advanced care planning is a chance to look at “what ifs” related to your health. It offers you protection in the event of a medical emergency or life threatening illness when you are unable to speak for yourself. Advanced care planning also offers you and your loved ones peace of mind. We know this planning process can be an overwhelming one – VACIHCS IS HERE TO HELP! Although some might be prompted to make plans after a health diagnosis, loss of a loved one, or family change – all adults are encouraged to start these critical conversations. VACIHCS is now hosting Advance Care Planning via Group Visits which fosters productive discussion with health care providers, Veterans, and their loved ones. Participants have enjoyed the group setting as it’s allowed Veterans to share experiences and plan for the care they desire.
