To speak with someone in compensation and pension, call the Medical Center at (515) 699-5888.
Release of Information
If you need to get information from your medical records, please call (515) 699-5802. You can also fax a request for release of information to Fax (515) 699-5934. A request can also be mailed to 3600 30th St Room 115, Des Moines, IA 50310. Office Hours to request in person are 08:00 a.m-4:00 p.m Monday through Friday. You will need the following form VA 10-5345A Form to have your personal records released to you. If you need medical records released to someone else other than yourself, utilize Form VA 10-5345. Both forms are fillable. Please fill them out, print the appropriate form; fax, mail or bring into the office.
